Your Initial Visit

If you're preparing for your child's first visit, you can do a few things to help expedite the appointment. Your child's complete immunization records need to be provided prior to your first appointment. Please contact our
nursing staff with this information.
Please provide the following information when you arrive for your visit:
- Insurance cards
- Medical records from your previous pediatrician
- Complete immunization records (please provide prior to your first appointment)

New patients, please complete all forms in full, sign and date as directed. One Registration form can be used per family. The other medical history forms will need to be completed in full for each child.
Current patients with address, phone number or other changes in demographics, please print and complete a new registration form in full, sign, date, and bring to your next appointment.
